You can use this form if you have received a message from the Migration Agency asking you to make an appointment.
The embassy will contact you within 3 working days after receiving this form.
Attention! You must be present at the front desk 15 minutes before your scheduled appointment time. If you do not arrive on time for the interview or do not notify the embassy of any obstacles, you may be rescheduled to a later date!
