6: What documents are needed when applying for a coordination number (samordningsnummer)?
When applying for a coordination number for children, the Swedish citizenship must be verified for at least one parent. The child and at least one parent must be present at the time of application.
You need the following documents in original:
- Both parents' passports
- The child's Slovak ID-card or registration certificate showing where the child is registered (not older than three months)
- The child's international birth certificate
- Childbirth or Pregnancy Certificate
- Form SKV 7750 from the Swedish Tax Agency
- Marriage certificate, or paternity confirmation if the parents were unmarried at the time of birth
- The form "Information for verification of Swedish citizenship".
