The following documents in original are needed when applying for a passport/ID-card for adults:
- Valid passport
- The form "Information for verification of Swedish citizenship"
- Slovak ID-card or registration certificate or Austrian "Meldebestätigung" or German "Meldebescheinigung" or other proof of residency in the country you live in
- An international birth certificate if you are born outside of Sweden and have never been registered in Sweden
- In some cases, you also need a certificate that you have retained Swedish citizenship, or a citizenship declaration from the Swedish Migration Agency
- In some cases, the embassy may require additional documents to establish citizenship
The following documents in original are needed when applying for a passport/ID-card for children:
- Valid passport for the child
- Both parents' valid passports
- The forms "Guardian's certificate" and "Information for verification of Swedish citizenship"
- An international birth certificate if the child is born outside of Sweden and has never been registered in Sweden
- Slovak ID-card or registration certificate or Austrian "Meldebestätigung" or German "Meldebescheinigung" or other proof of residency in the country you live in

Please note that People with a coordination number (samordningsnummer) must count on a longer processing time (4-6 weeks after application) to get their passport, if the number has been declared dormant by the Swedish Tax Agency. Unfortunately, the embassy has no influence on the processing time. See more information at Skatteverket.se
